That was going to be the whole medical record, and then I was going to sit down for first pitch like a man who had been scanned.
You know this move. The body is quiet, the scale has already done something impressive, and you let that quiet sign a chart. I am 40. I am a day-shift hospital security supervisor, nights behind me, 308 to 196 over 9.5 months starting mid-2025. None of that came with a picture of an artery. Bundgaard, García-Lunar, Kofoed, and the REACT investigators published this online August 29, 2026, in the New England Journal of Medicine. Denmark and Spain. 16,808 adults, ages 18 to 70, no known atherosclerotic cardiovascular disease. They did not ask who felt fine. They imaged the carotid and femoral arteries with three-dimensional vascular ultrasound, and they added coronary computed tomographic angiography. The Novo Nordisk Foundation paid for the pictures. It is a prevalence study, not a treatment study, so I do not get to turn it into a dose.
Silent atherosclerosis was already there in 57.1% of them. The 95% confidence interval was 56.3 to 58.0. Mean age 45, standard deviation 12. Women were 51.4% of the room. I am 40, so I am in the band, and I do not have their pictures. More than half of a group picked for having no known disease already had plaque. That is the door I was about to walk through this morning. Feeling fine, no diagnosis, lighter Saturday, done.
I tried the young line as a permission slip. Ages 18 to 29, 8.7% of the men and 6.7% of the women. I am 40. I almost texted that screenshot to myself. It would have been embarrassing and also wrong. Prevalence rose after that stratum, and women had a later, steeper climb in midlife. Their background line is the one that finished me: silent atherosclerosis tracks with cardiovascular events and death independent of traditional risk factors. The scale moved. The pipes did not send a postcard.
No symptoms is not a clean result.
I had been checking the wrong door
I wanted the plaque, if it existed, to be a chest story, so a quiet chest could acquit me before the game. Isolated coronary atherosclerosis was the uncommon pattern. So the test I trusted was the pattern they barely found. If your only open hour is after first pitch, you are in this kitchen. If your watch already closed Friday, you are holding the same quiet chest up to the light and calling it a scan. The sample is Denmark and Spain. The door is not.
